  • Right to Amend: If you feel the information we have is incorrect or incomplete, you may ask us to amend it. We may deny your request if the information was not created by us or is already accurate.
  • Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, or operations.
  • Right to Request Restrictions: You have the right to request a restriction on how we use or disclose your PHI. We are not required to agree to your request, except in the case of a disclosure to a health plan for a service you paid for entirely out-of-pocket.

  • Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location (e.g., calling a cell phone instead of a home phone).

  2. The Secretary of the U.S. Department of Health and Human Services.

We will not retaliate against you for filing a complaint.
