At Portside Dental, we are committed to protecting your medical and dental information. We are required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this notice of our legal duties and privacy practices.
1. Uses and Disclosures of Health Information

- Payment: We may use and disclose your information to obtain payment for the services we provide. This includes contacting your insurance company to determine eligibility or to submit claims.
- Healthcare Operations: We may use your information for the internal operations of our Traverse City office, such as quality assessment, practitioner reviews, and business planning.

2. Other Permitted and Required Disclosures
We may disclose your PHI without your authorization in the following domestic and legal circumstances:

- Abuse or Neglect: To government authorities if we believe a patient is a victim of abuse, neglect, or domestic violence.
- Legal Proceedings: In response to a court order, subpoena, or discovery request.
- Law Enforcement: To law enforcement officials for specific purposes, such as identifying a suspect or missing person.
3. Your Rights Regarding Your PHI
You have the following rights regarding the health information we maintain about you:
- Right to Inspect and Copy: You have the right to inspect and receive a copy of your dental and billing records. We may charge a reasonable, cost-based fee for copies.
- Right to Amend: If you feel the information we have is incorrect or incomplete, you may ask us to amend it. We may deny your request if the information was not created by us or is already accurate.
- Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, or operations.
- Right to Request Restrictions: You have the right to request a restriction on how we use or disclose your PHI. We are not required to agree to your request, except in the case of a disclosure to a health plan for a service you paid for entirely out-of-pocket.
Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location (e.g., calling a cell phone instead of a home phone).
Our Duties
Privacy Maintenance: We are required by law to maintain the privacy of your PHI and to provide you with this notice.
- Breach Notification: We will notify you following a breach of unsecured PHI as required by law.
- Adherence to Terms: We are required to abide by the terms of the notice currently in effect. We reserve the right to change this notice and make the new provisions effective for all PHI we maintain.
5. Complaints
If you believe your privacy rights have been violated, you may file a complaint with:
- The Privacy Officer at Portside Dental.
- The Secretary of the U.S. Department of Health and Human Services.
We will not retaliate against you for filing a complaint.
